Registration Forms
If you have any difficulty downloading the forms, move the mouse cursor over the form you wish to download, then use the right click button on your mouse. This should pop-up a menu. Depending on the browser you use, you will see an option like "Save Target As .." or "Save Link As ..".Download appropriate form. Incomplete forms will not be accepted.

ALL COACHES, MANAGERS AND UMPIRES MUST COMPLETE THE APPROPRIATE REGISTRATION FORM AND THE PROHIBTED EMPLOYMENT FORM (Child Protection). PLEASE ENSURE FORMS ARE COMPLETED IN FULL AND SIGNED. PLEASE PRINT CLEARLY.
- No player will be registered without a completed registration form signed by either the parent (under 18) or player (over 18) with payment in full.
- Late registrations will only be considered if submitted on a Registration Form with full payment and there is availability in an appropriate team.
ALL FEES MUST BE PAID IN FULL BY 23 FEBRUARY.
- New players must provide (with their registration form), one (1) passport size identification photograph and a copy of birth certificate or passport as proof of age.
- One (1) new passport photo is required for all juniors who have played for 3 years or more and MUST BE included with your registration form.
REGISTRATIONS WILL NOT BE ACCEPTED WITHOUT THE PASSPORT PHOTOGRAPH OR PROOF OF AGE IF APPLICABLE.
